EMT Application Checklist

Use this EMT application checklist as a guide for successfully completing the application process for the college and the EMT program. All application materials must be received on or before January 15 or July 31 of each year(if deadline falls on a weekend or holiday, deadline will be Friday before).

Step 1: Complete Chattanooga State Community College Enrollment

  • For consideration of program admittance, you must have completed Chattanooga State enrollment before the program deadline.
  • If you are not a current student at Chattanooga State, please complete a college application online and choose "Pre-Allied Health" as your planned course of study. Apply Here
  • Completion of all admission requirements to Chattanooga State entails:
    • Submitting all official transcripts, vaccination, proof of citizenship, etc.
    • Testing (if applicable)
    • Orientation (if applicable)
    • Go to the admission status page to check on your Chattanooga State admission status.

Step 2: Meet with an Advisor to Develop Your Academic Plan

  • Please call (423)697-2456 to set up an advising appointment.  All learning support classes should be completed first (if applicable).
  • If you are college level go to the required EMS Program Information Session. You will receive important information on the program acceptance.

Step 3: EMT Admission Process

  • The EMT Training Program follows the 2009 National Highway Traffic Safety Administration National EMS Education Standards. 
  • Additional materials and skills required by the Tennessee Department of Health and Environment and Division of Emergency Medical Services are included.
  • Applicants must be 18 years of age or older and possess a high school diploma, GED, or HiSET.
  • Complete Chattanooga State College Enrollment.
  • Complete all learning support classes (if applicable).
  • Attend a required EMS Information Session.
  • Submit EMT Program Application and Required Materials by the program deadline dates of January 15 or July 31 of each year.
  • Review the EMT Application Checklist to successfully complete the process.
  • Complete the Online Allied Health EMT Application form. Upload all required documents.

Note to applicants:
EMT applicants are selected on a first come-first serve basis.  It is your responsibility to complete the Chattanooga State admissions process in order to get registered for an EMT spot. 
Failure to complete the required process for the EMT program in the approved manner will result in your slot being assigned to another student.
 
 
Copy of your high school transcript or GED/HiSET
Note:  All previous transcripts sent to the Chattanooga State Admissions Office cannot be copied and forwarded to the Nursing & Allied Health Division.  Your high school should send an original to the student to make a copy.  Copy of the Information Session Student Information Form after completing the online EMT information session.
Submitting the online application will automatically send your application to the Allied Health Technical Clerk
