Sonography - Application Checklist

Use this Sonography application checklist as a guide for successfully completing the application process for the Sonography program. All application materials must be received on or before May 15, 2026.

1. Apply to Chattanooga State Community College
 

2. Degree Requirement 

To be eligible to apply to the Sonography Programs at Chattanooga State Community College, applicants must have either an Associate of Applied Science degree, or a bachelor’s Degree. All levels of degree must be completed by May of the intended application year. 

  • Option 1) An Associate of Applied Science (A.A.S) degree in a Health Sciences area that is patient‐care related with clinical rotations. Including but not limited to Dental Assisting, Dental Hygiene, Medical Coding and Billing, Medical Laboratory Technology, Nuclear Medicine Technology, Nursing, Paramedic, Occupational/Physical Therapy Assistant, Ophthalmic Medical Technician, Radiologic Technology, Respiratory Care, Sleep Diagnostics Technology, Surgical Technology, or Veterinary Technology.
     
  • Option 1.1) An Associate of Science (A.S.) degree in a Health Science/Allied Health/Science major. Including but not limited to Biology, Chemistry, Physics, Exercise Science, Kinesiology, Health Science, or Imaging Science
     
  • Option 2) A bachelor’s degree (any major is accepted)
     

3. Required Prerequisites 

Complete all prerequisite courses by May of your intended application year  
A letter grade of a “C” or higher is required to be considered competitive  

  • Human Anatomy & Physiology I 
  • Human Anatomy & Physiology II 
  • Medical Terminology 
  • Survey of Physics or Principles of Radiation Physics
  • College Algebra, Pre-calculus Algebra, or Statistics

 

4. Completion and on time submission of all required application documents: 

  • Sonography Application (complete online)
  • Copy of all previous college *transcripts
    • Use the N&AH Transcript Request Form on Sonography website to obtain transcripts for the program.
    • Copies are needed from all previous college(s)
    • Do not request transcripts from the Records Office at Chattanooga State.
       
  • Clinical Observation Evaluation Form (must complete at least 16 observation hours)
  • Health Program Clinical Instructor Reference Form (only for A.A.S. degree applicants)
  • Health Education Program Director Reference Form (only for A.A.S. degree applicants)
  • Personal Reference Form (professional in nature)

All application materials should be attached to your CVS Sonography Application (available via TigerWeb January 15,2026 – May 15, 2026.
